Nursing-home-level care for adults with cognitive impairment or decline

At-home nursing care is not only possible — it’s a game-changer

Adults who are eligible for nursing home services may also be eligible for top-notch care at home via the Nursing Home Transition and Diversion (NHTD) Waiver. Similarly, adults who have suffered strokes or other traumas may qualify for assistance via the Traumatic Brain Injury (TBI) Waivers.

For caregivers who are stretched too thin, these waivers, administered via the New York State Medicaid program, can offer the relief they desperately need.

Who is eligible?

Participants must meet these criteria to apply for this service via Hamaspik of Kings County.

  • Live in NYC or Nassau County
  • Need nursing-home-level care and have a cognitive impairment (for NHTD)
  • Ages 18+ (TBI approved up to age 65)
  • Have a cognitive impairment, including a disorder due to a traumatic brain injury, such as a stroke (for TBI)

Once approved, you may be eligible for:

  • Service coordination, which can be formidable for any caregiver, is a helpful part of the service. A coordinator helps write the care plan, access services, and ensures they are in place and working effectively.
  • To keep people safe at home and in their familiar communities for as long as possible, certain home modifications and equipment may be funded. This can include anything from ramps to stairlifts to essential repairs.
  • One of the biggest perks of the NHTD and TBI programs is the additional caregiver hours and shifts that are not generally available via other channels. 
  • Once approved for NHTD, you get top-of-list priority for Section 8, a need-based NYS program that subsidizes rent.

"When Miri's mother, matriarch of a large family, was diagnosed with Alzheimer's disease, all her children rallied around her. Her daughters and daughters-in-law took turns cooking and spending the day with their mother, arranging to have a grandchild sleep over each night.

However, as time went on, her condition deteriorated, and it became more and more difficult to care for her on their own. Miri's mother would wake up at night disoriented, wanting to wander off, and the grandchildren were not equipped to handle this. They were all being stretched thin and it was clear the support system they had set up was no longer sustainable.

Miri took up the daunting task of trying to secure an aide. However, despite her tireless efforts, the bureaucratic maze seemed insurmountable. The family faced the looming question —was placing their mother in a nursing home the only option?

It was Yossi, Miri’s youngest brother, who discovered the NHTD Waiver — and the hope that they could receive the support they needed to keep their mother home. Upon reaching out to Hamaspik, Miri was connected to a service coordinator who worked tirelessly to get her mother approved. They were so relieved to receive full-time care and home modifications allowing their mother to age peacefully at home, surrounded by her loved ones."

To be eligible for NHTD/TBI, you must apply for both service coordination and HCSS (home care). This process can take upwards of six months.
