A support system for adults experiencing emotional or mental health challenges

It’s a lot to deal with

When your mental or emotional health isn’t good, every single aspect of your life is impacted. Not only do you need to manage your condition, but you need to do that within the ever-changing limitations you find yourself up against. It’s a lot.

There’s only so much a person can hold at once. Even with supportive family and professionals, sometimes you need an experienced, compassionate outsider to step in.

That’s why Integrated Health offers a support system. This support system gives you access to resources that will help you manage your condition. It can also provide you and your family with respite and other services to facilitate your recovery.

Care Management eligibility criteria

  • Resident of NYC, or Nassau
    or Suffolk Counties
  • Age 18+ (Care Management )
    Age 21+ (HARP)
  • Have an emotional or mental
    health challenge

Integrated Health is a program certified through the New York State Department of Health. It meets the acute need for better mental health care in our communities.

— the full rehabilitation of the person within their familiar community.

We’re bound by HIPAA and we won’t take any steps without your consent. You get to list the people you want on your care team, and you decide which services will be helpful.

She leads the team in providing the highest standard of care for every client, and is at the forefront of the continued expansion and growth of Sipuk Clinic and its services.

With over 20 years of clinical experience, Barry is an expert in the areas of trauma and abuse. He is a noted speaker and author whose weekly training sessions and ongoing supervision of adult therapists foster a culture of excellence and informed practice.

What Care Management
can do for you

You may be eligible for a wide array of support, including:

Sourcing specialized doctors, therapists, and providers who have a strong reputation for treating your specific condition

Steady emotional support
so you’re not left alone to deal with side effects and heavy emotions between appointments

Help scheduling and keeping appointments, including transportation if needed

Assistance with discharge
in the event of hospitalization, as hospitals often require that you arrange for continuity of care

Help applying for social services programs, like SNAP, SSI, HEAP, Section 8, childcare vouchers, and other benefits

Referrals to interesting programs so you can study something you always wanted to learn, or employment assistance to help you find a job that is a good fit for your talents and gives you the flexibility you need right now

Help from HARP that
you may appreciate

If you’re eligible for HARP (Health and Recovery Plan) you can receive:

At-home therapy provided by a licensed therapist so you can be treated where it’s most comfortable for you

Coaching in household and life skills, including organization, time management, marital and parenting skills, to help you create a warm, orderly home environment that promotes healing

Healing social and recreational activities, like art and music, provided under clinical supervision to ease your journey to recovery

Daily support from a DSP (Direct Support Professional), or worker, to continue rehabilitation treatment at home or in the community setting of your choice

Family support from a professional who can explain your struggles to your family so they can be there for you

Peer support so you can get validation and encouragement from someone who fought a similar battle and triumphed. You’ll pay it forward someday!
